About Our Organization
The Center for Hospice & Palliative Care is dedicated to providing seriously ill individuals, and those who share their lives, with exceptional care. This care includes hospice, palliative care, grief support, advocacy and other life-enhancing services. The Center consists of the following agencies:
- Hospice Buffalo provides hope, care and dignity to patients who face a life-threatening illness and their families.
- St. John Baptist/Hospice Buffalo House is the first of its kind in the nation. St. John Baptist/Hospice Buffalo House is a place of comfort and care for patients in their final months or weeks of illness. This eight-bed House provides a serene home-like environment for Hospice patients who are unable to remain at home.
- Home Care Buffalo/Essential Care for Children provides comprehensive support to children with a life-threatening illness and their families. Essential Care's services are provided in Erie & Niagara counties.
- Life Transitions Center, Inc. provides counseling, support and education to children, adults and families concerned with grief, death and coping with a serious illness.
- Hospice Foundation of WNY accepts, administers and invests all charitable funds on behalf of The Center for Hospice & Palliative Care and its affiliate agencies.
